Our contact form is for general enquiries only. Please do not use it to send protected health information, describe symptoms, request clinical advice, or discuss your care.

portal, which is designed for secure communication.

you consent to the jurisdiction of those courts.

If we cannot resolve a dispute informally within thirty days, either party may pursue the matter in the courts identified in Section 14.
