Effective February 1, 2026, there is a charge of $35 per form for the completion of FMLA and Disability paperwork.
Office Closure for Independence Day Holiday
The Women's Group will be closed Friday, July 3 in observance of the Independence Day holiday. We will be open for regular business hours on Monday, July 6.
If you have an urgent matter, you may reach our on-call nurse and physician by calling our main office number (850) 476-3696. Routine matters and questions should be handled during regular office hours.
