Effective February 1, 2026, there is a charge of $35 per form for the completion of FMLA and Disability paperwork.
Office Closure for Memorial Day Holiday
The Women's Group will be closed Monday, May 25 in observance of the Labor Day holiday. We will be open for regular business hours on Tuesday, May 26.
If you have an urgent matter, you may reach our on-call nurse and physician by calling our main office number (850) 476-3696. Routine matters and questions should be handled during regular office hours.
